Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications, treatments, and skilled nursing procedures in a home care setting
- Monitor and document patient progress, vital signs, and changes in condition
- Coordinate care with physicians, therapists, and other healthcare team members
- Educate patients and family members on disease management, medications, and home safety
- Perform wound care, IV therapy, and other specialized clinical interventions as needed
- Ensure compliance with state, federal, and agency regulations and documentation standards
