RN Home Health Care Per diem

Duties * Conduct thorough patient assessments, including vital signs, physical examinations, and functional evaluations to develop personalized care plans. * Educate patients and their families on disease management, medication administration, nutrition, and safety measures to foster independence. * Valid RN license in the state of practice with current BLS (Basic Life Support) certification. * Proven experience working in home health settings or similar environments such as hospitals, nursing homes, or assisted living facilities. * Knowledge of HIPAA regulations to ensure patient confidentiality and privacy at all times. * Experience working with diverse populations including pediatrics, geriatrics, individuals with disabilities or developmental challenges, and memory care patients. * Ability to perform physical assessments and vital signs accurately while demonstrating excellent communication skills. * Critical care experience preferred—especially ICU or trauma center background—and familiarity with ICD coding (ICD-10/ICD-9).
