Case Manager- Supportive Housing Tuesday-Saturday 12:00pm-8:00pm. POSITION SUMMARY/FUNCTION Responsible for providing care coordination / case management services to those with mental illness, substance use and/or developmental disabilities. Conduct interviews and assist in accessing appropriate community services. Maintain appropriate patient records and tracking of patients between agencies. Some travel may be required within the catchment area. ESSENTIAL DUTIES/RESPONSIBLITIES/FUNCTIONS/TASKS: Provide initial care coordination/case management services and develop written service plans for each patient. Conduct face to face interviews (where appropriate) with patients and/or patient representative to assess individual needs, supports, goals, etc. Information gathered may include but not limited to: identifying information, community living, employment, physical needs, health, social history, housing and physical environment, socialization and recreation. Identify appropriate community service needs of patients and determine appropriate community resources to meet those needs. Involve the patient and/or family in the development of the written service plan and conduct in a timely manner. Effectively implement service plans for patients. Coordinate service delivery to patients among appropriate agencies as specified in the service plan. Provide family support and/or basic living skills as needed. Conduct periodic assessment and modification of service plan. Advocate on behalf of patient and target population as a whole regarding social, economic, and residential requirements. Conduct home visits. Provide transportation only when it is a necessity. Ensure all community resources such as family, friends, public transportation and special vehicles are utilized to meet the needs of patient. If transporting clients in personal vehicle, must maintain additional liability insurance as required. Coordinate care for patients between agencies and monitor/follow up with the patient and agency regarding this care. Close cases as appropriate. Meet required productivity level of 80% direct service time. Assist with maintaining patient records in a professional manner and in accordance with all SpectraCare policies and procedures, Alabama Administrative Code, CMS, and all other applicable regulating and contractual entities. Ensure privacy and confidentiality of all patient records in accordance with HIPAA, 42 CFR and all other applicable state or federal laws. Ensure all written documentation is completed accurately and in a timely manner. Ensure that all records are staffed within standard time frames, with service plan revised as needed and in accordance with regulatory requirements. Ensure that all progress notes are completed promptly, written in a professional manner and in accordance with policies and procedures. Ensure that goals are appropriate, specific, and the focus of treatment. Ensure that all patient information is kept up to date. Ensure patient release of information forms are signed in a timely manner and maintained within the chart. Courteous and respectful towards patients, visitors, and co-workers. Treats patients with care, dignity, and compassion. Respects patient’s privacy and confidentiality. Is pleasant and cooperative with others. Assists patients and visitors as needed. Personal values do not inhibit the ability to relate and care for others. Is sensitive to the patient’s needs, expectations and individual differences. Is gentle and calm to patients and families. Administrative and other related duties as assigned to include but not limited to: Serve as a liaison and advocate for SpectraCare patients with community agencies and the public. Assist with ensuring the care coordination/case management services remain within budget parameters. Actively participates in committees as required. Completes assigned tasks in a timely manner. Works in a cooperative manner with other SpectraCare employees. Receive and respond to inquiries promptly and courteously. Must perform all duties with the highest standard of ethics. Attends regular workshops, trainings, in-services, and/or administrative meetings as appropriate. Maintain travel records accurately and submitted within the appropriate timeframe. Ensure necessary documentation is completed and up to date before accrued leave is taken. Adhere to Dress Code as set forth in the SpectraCare policies and procedures. Follow guidelines of SpectraCare Personnel Policies, SpectraCare Clinical Policies, Alabama Department of Mental Health Administrative Code, CMS, and all other applicable regulating and contractual entities. Performs duties independently and/or with minimal supervision. Functions within professional boundaries with patients and others. Works well with other staff and patients to ensure safety of program, patients, and other staff. Functions and performs well in stressful environments and manages work related stress to ensure client safety. Demonstrates a willingness to work on any unit/program of SpectraCare based on facility needs as directed. Conforms to SpectraCare’s Core Values as duties are carried out daily. Other services, tasks, functions, and responsibilities deemed necessary; not otherwise specified. Fulfill duties of Nurse Delegation Program. Become fully certified as a Medication Administration Certified (MAC) assistant. Follow all regulations as outlined in the MAC policies including but not limited to the Seven (7) Rights and Responsibilities, training, consultation with RN/LPN, error reporting, etc. Comply with all documentation requirements of the MAC program including the MARS, counting and recording controlled substances, PRN medications, sign out sheets for home visits, and medication errors reporting sheets. Regular attendance with timely arrival and departure according to scheduled hours. Must be able to work full time with rotating assignments as necessary to include but not limited to overtime hours, nights, and/or weekends as scheduled. To ensure client/employee safety while on duty, must remain awake during scheduled working hours. Comply with SpectraCare’s attendance policy. NON-ESSENTIAL DUTIES/RESPONSIBLITIES/FUNCTIONS/TASKS: Attend mandatory trainings as required by ADMH or other agencies. Attend departmental meetings and other assigned meetings. Attend all required job-related/system training. Assist in other areas as needed. Conforms to System’s policies and procedures. ESSENTIAL QUALIFICATIONS/KNOWLEDGE/SKILLS/ABILITIES: Knowledge of developmental, substance use and/or mental illness disorders. Ability to design, implement/administer individualized behavioral and/or developmental services in regard to case management services. Ability to provide services in all facilities and other locations as deemed necessary. Must be motivated and able to make quick and accurate decisions. Must be able to meet and exceed expectations. Must be able to perform all duties with minimum mistakes and without constant supervision. Ability to effectively present information and respond to questions from groups of supervisors, managers, and/or the general public in an effective and understanding manner, to promote quality of care. Ability to organize and manage multiple priorities with minimal supervision. Ability to maintain unit files to include but not limited to patient charts, case management documentation, etc. in accordance with the requirements of SpectraCare, Alabama Administrative Code, CMS, and all other applicable regulating and contractual entities. Ability to attend to detailed assignments and follow through with task until completion. Ability to perform all duties in a safe manner, without endangering any employee or asset. Working knowledge of business English, spelling and math. Working knowledge of office practices and procedures. Skill in keying rapidly and accurately. Ability to provide excellent verbal and written communication skills. Ability to communicate with patients, families, and/or other agencies regarding case management services. Ability to adhere to health and safety regulations. Ability to adhere to duties assigned by Supervisor and/or other designees. Ability to operate office machines such as computers, copier, fax machines and/or other equipment or programs. Ability to make minor decisions in accordance with rules and regulations and apply these to work problems. Ability to understand and follow moderately complex oral and written instructions. Ability to maintain confidentiality and to establish and maintain effective working relationships with other employees and the public. Able to work in a fast-paced environment. Ability to work independently and/or in a team environment as required. Ability to work in a stressful environment. Ability to travel within catchment areas via personal transportation and/or system transportation as required. Ability to get along with others and manage work related stress to ensure client/staff safety. Ability to adapt to conditions which may vary considerably and/or to respond professionally and efficiently in crisis situations. Ability to pass a drug screen when given to include but not limited to pre-employment, random, post-accident, suspicion, etc. Ability to maintain professional boundaries with patients i.e., not crossing professional boundaries to include but not limited to fraternization. ESSENTIAL TRAINING REQUIREMENTS: Complete initial new hire training. Complete required on the job training (OJT) training by the department. Obtain proper certifications needed to perform job duties. Complete annual training as required by the Alabama Administrative Code, CMS, and all other applicable regulating and contractual entities. Complete Care Coordination / Case Management Training as required by the Alabama Department of Mental Health If working in the Developmental Disability Division, for purposes of facilitating Person-Centered Planning meetings and service coordination, QDDP must have at least one year of experience working directly with persons with developmental disabilities, have successfully completed a four-day Personal Outcome Measures (POM) workshop and co-facilitated a Person-Centered Planning meeting with ADMH staff. Obtain TB skin test and/or TB Assessment prior to initiation of duties after hiring and annually thereafter. Complete First Aid/CPR upon hire and working alone with clients; and continuously thereafter maintaining current certification. WORK ENVIRONNMENT / PHYSICAL REQUIREMENTS: Physical ability to work assigned shift. May require periodic weekend/evening work to meet the organizations’ needs. Must be able to travel to system facilities and/or other locations/outings with exposure to changing weather conditions. Work location is primarily in a temperature-controlled setting with minimal periods of sitting at a desk with associated repetitive motions. Must understand and practice good ergonomic movements to avoid injuries. Physical ability to correctly lift and/or moves up to fifty (50) pounds on an infrequent basis. Required to talk to or hear staff, individuals served, and/or others. Required to stand, walk, sit, bend, twist, kneel, and/or reach frequently. Physical ability to perform CPR. Employee may be exposed to one or more of the following: disagreeable odors, chemicals such as disinfectants, cleansers, soaps, etc; latex, plastic and other materials used for personal protective equipment; noise and distractions; unpleasant sights; unpredictable behaviors; contagious diseases; and/or wet floors. Required to train professional staff through verbal and visual presentations.
