About this role
RN Case Manager, Home Health. James River Home Health is seeking a compassionate and dedicated Home Health RN Case Manager to support patients in the Prince William territory. In this role, you will manage a caseload of patients in their homes while collaborating with the interdisciplinary team to ensure a seamless, high-quality care experience. Provides skilled nursing care to homebound patients, coordinates and manages multidisciplinary services, educates patients and families on health maintenance and disease prevention, and offers guidance and emotional support throughout the care journey.
Requirements
- Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia
- At least one year of home health experience preferred
- Graduate of an accredited nursing school
- Must have a valid driver's license and a reliable car available during work hours
- OASIS and HCHB experience PLUS!
