THE POSITION
As a Community Health Nurse with the Department of Health (DOH), you will help strengthen and support healthier communities throughout the Southwest District. This role gives you the chance to apply your expertise in public health and education while partnering with dedicated colleagues and engaged community organizations. Your work will lift up vital programs that protect health, expand access, and respond to emerging needs across the region. DESCRIPTION OF WORK
This role focuses on assessing community needs, planning and delivering public health services, and supporting local health initiatives. Your work helps strengthen public health programs across the Southwest District. As a Community Health Nurse, you will perform the following
duties:
Community Assessment: Analyze county level health data to identify needs and resources
Program Coordination: Link program resources with community partners to support local health activities
Initiative Development: Develop public health initiatives using protocols, guidelines, and community data
Disease Monitoring: Monitor communicable and vaccine preventable diseases using public health information systems
Health
Education:
Provide education programs that address a variety of public health topics
Clinical Services: Deliver clinical services for immunizations and specimen collection to promote patient health and prevent or control disease

Work Schedule and Additional Information:
Full-time employment, 37.5 hours per week
Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
Irregular
Schedule:
Work infrequently requires statewide travel and irregular/flexible work hours including overnights, weekends, and holidays in accordance with policy.
Telework: You will not have the option to telework in this position.
Salary:In some cases, the starting salary may be non-negotiable.

QUALIFICATIONS
REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY
Special
Requirements:
You must possess active authorization to practice as a Registered Nurse in Pennsylvania.
All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.

You must be able to perform essential job functions.

Requirements:
You must pass a background investigation.
This position falls under the provisions of the Child Protective Services Law.
Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
