Registered Nurse (RN) – Hospice Lakewood Hospice and Palliative Care, Inc. is looking for an experienced and compassionate Registered Nurse (RN) to join our hospice team serving patients throughout the Denver metro area and surrounding communities.
We are seeking a clinician who understands the unique needs of hospice patients and families and is committed to providing excellent clinical care with compassion, dignity, and respect.
Qualifications
• Current, unrestricted Colorado RN license
• Hospice, palliative care, home health, or related experience preferred
• Strong clinical assessment and critical-thinking skills
• Excellent communication and patient/family education skills
• Ability to work independently while collaborating with an interdisciplinary team
• Valid driver's license and reliable transportation
Responsibilities
• Provide skilled nursing care to hospice patients in their homes
• Perform comprehensive nursing assessments and develop individualized plans of care
• Monitor symptoms and coordinate appropriate interventions
• Educate and support patients, families, and caregivers
• Communicate with physicians and the interdisciplinary team regarding changes in patient condition
• Participate in care coordination and hospice plan-of-care updates
• Provide compassionate end-of-life care focused on comfort, dignity, and quality of life
