Registered Nurse (RN) – Start of Care and Home Health Visits | Kissimmee, FL. Matrix Home Care is seeking an experienced, compassionate, and dependable Registered Nurse to complete Start of Care assessments and skilled nursing visits throughout Kissimmee and the surrounding Osceola County area. Responsibilities include completing Start of Care assessments and initial patient evaluations, performing skilled nursing visits, reassessments, resumptions of care, recertifications, and discharge visits, developing and updating individualized plans of care, providing wound care, medication management, disease-management education, coordinating care with physicians and clinical team, and completing accurate OASIS and electronic medical record documentation.
