We're looking for a Director of Patient Care Services / Director of Nursing to lead our home health clinical team, ensure regulatory compliance, and drive quality outcomes that help patients heal safely in the place they love most. What you'll do: • Lead and manage all clinical operations • Supervise, mentor, and develop clinical managers and field staff, fostering a culture of collaboration and excellence • Ensure full compliance with Medicare, Medi-Cal, and state home health regulations, including Conditions of Participation • Oversee OASIS accuracy, care planning, and documentation to support quality outcomes and appropriate reimbursement • Partner with executive leadership on strategic growth, performance metrics, and process improvements • Serve as a clinical resource and trusted point of escalation for patients, families, and referral sources What you bring: • Must have: Active RN license in California (BSN required; MSN or equivalent preferred) • Must have: 5+ years of progressive nursing leadership experience in home health, including oversight of multi-disciplinary teams • Must have: Deep knowledge of Medicare home health regulations, OASIS, and PDGM reimbursement • Must have: Proven track record in survey readiness, quality assurance, and performance improvement • Must have: Exceptional communication and relationship-building skills with staff, physicians, and referral partners • Nice to have: Certification in Home Health Nursing (e.g., COS-C, CHPN) or Nursing Administration • Nice to have: Experience with EMR systems used in home health (e.g., Homecare Homebase, PointClickCare)
