Department 02193 GCMG Transition Services: Stanly - Transitional Care Clinic Status Part time Benefits Eligible Yes Hours Per Week 24 Schedule Details/Additional Information 24 hours per week- Tuesday and Thursday (clinic days) 3rd day undetermined Pay Range $35.50 - $53.25 Education Atrium Health Transitional Care Clinic- For 30 days following your release from the hospital, our dedicated team of doctors, pharmacists and social workers will provide the care and support you need. Our team will work with you to provide the resources, tools and education to help you have the safest, quickest and healthiest recovery possible. We can provide transitional care in our office, virtually or at your home, depending on your needs and available resources. We’re Looking For: Completion of an accredited or approved program in nursing Registered Nurse license issued by the state in which the teammate practices Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association One year of Nursing experience needed What You’ll Do As a skilled and compassionate RN, you will play a pivotal role in providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures. Using evidence-based practice, clinical decision making, compassion, and skills communication while leading efforts to create the safest patient environment and the best patient experience across the continuum. Requirements
