About this role
The primary purpose of your job position is to oversee the facility to ensure assessments are completed per guidelines. A primary focus is the resident assessment process, which includes individual assessment, MDS assessment, and resident plan of care. Essential duties include providing oversight on weekly PDPM meetings, tracking and monitoring PDPM tools, QM and QASP performance, providing PDPM and MDS training, assessing new admissions, ensuring accurate and timely completion of MDS Assessments, coordinating interdisciplinary participation, maintaining MDS assessment schedules, data entry and electronic submission, scheduling resident care conferences, assisting with care plans, evaluating care plans, managing level of care changes, and maintaining confidentiality.
Requirements
Graduation from an Accredited School of Nursing
Bachelor of Science Degree in Nursing preferred
Good standing with State Board of Nursing
Maintain all required continuing education/licensing requirements
3 years of supervisory experience preferred
Long Term Care Experience preferred
Proficient in Microsoft Suite products
Knowledge and experience with PCC preferred
