Hours: 40 hours per week with work schedule option and rotating weekend shifts Monday - Friday, 8:00 am to 4:30 pm or 8:30 am to 5:00 pm OR Monday - Thursday, 8:00 am to 5:30 pm (9 hours) and Friday, 8:00 am to 4:30 pm (8 hours), off every other Friday As a Nurse - LVN, you will: Consult with Program psychiatrist for medication orders and guidance/recommendations for the treatment of medical and psychiatric conditions. Recognize accepted usage and side effects of ordered psychotropic and obtain treatment for side effects if present. Monitor clients physical and psychiatric condition and response to treatment and/or medication. Report clients' responses to the Program psychiatrist. Maintain inventory, storage and handling of medication including Patient Assistance Program (PAP) medications and/or sample medications according to Medi-Cal and all overseeing agency's guidelines. Maintain controlled medications securely and separately from non-controlled drugs. Complete and sign all required medical/nursing documents. Provide health education and medication instructions to clients, family members, staff and other support personnel. Provide services in the community or clients home, including administration of medication Identify acute or potential health problems and refer clients to the appropriate facility and/or level of care for treatment. Our Intensive Mobile Psychosocial Assertive Community Treatment programs (Downtown IMPACT, IMPACT, Senior IMPACT and South Bay IMPACT) provide intensive, community-based case management services for homeless adults throughout San Diego County. We deliver fully integrated services to clients diagnosed with a serious mental illness, as well as individuals with co-occurring, mental health, and substance disorders. Our programs utilize the evidence-based practice of Assertive Community Treatment (ACT) to support our clients in the higher level of community care and to assist them in reaching their goals and living a life not dominated by mental illness. Our team is a model of care, providing treatment, rehabilitation, and case management supports. Intravenous Therapy and Blood Withdrawal Certified at time of onboarding or must be obtained within 90 days of hire. Credentialing: If you are applying to this position as a Licensed, Registered, Waivered, or Certified professional and are offered the position, you will be required to complete the credentialing process. Additional information will be provided at the post-offer stage. Additional Qualifications and Skills Minimum of one year of experience working with a mental health population, especially with individuals with severe and persistent mental illness and co-occurring disorders is strongly preferred Ability to physically and mentally able to assist in emergency situations is required Proficient computer skills, including MS Office This program is housed in a two-story building that does have an elevator; however, ability to navigate stairs may be required. A valid California driver's license in good standing, personal vehicle, proof of insurance and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position Ability to pass criminal/background clearance Under the direction of the Nurse Coordinator delivers behavioral health nursing care. Ensures health care needs of clients are addressed using the nursing process and according to CRF Policies and Procedures. Assists clients in accessing community medical and psychiatric resources to achieve optimal level of health and functioning in the community.
