About Hospice
Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. Scope of Work
The RN is responsible for assessing, planning, implementing, coordinating, monitoring and evaluating the strategies and services required to meet the patients/family's end-of-life care needs. An outcome based plan of care is created and continually adjusted through utilization of the nursing process, relationship centered care principles, and holistic philosophy, assuring that the changing needs of the patient/family are met while facilitating a pain-free, peaceful, sacred death.
Essential Functions
Exhibit “Patients and Families First” attitude. Responds with compassion, flexibility and urgency to each patient’s needs. Provides the care that patients want when they want it.
Develop a comprehensive plan of care that addresses physical, emotional, cultural and spiritual needs of the patient/family with input from the patient/family and the interdisciplinary team. Evaluate this plan and makes interventions as necessary.
Advocate for pain/symptom management.
Educate and provide the support to patient/family regarding care issues. Committed to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service. Adheres to the organization’s Mission, Service Standards and Code of Conduct.
Direct and supervise LPN and CENA.
Administer and document medications/treatments as ordered by the physician according to policies in an accurate and timely manner.
Identify and respond appropriately to emergency situations.
Attend shift reporting and IDT meetings as necessary to facilitate communication of decisions related to the patients care.
Actively participates in safety initiatives and risk mitigating measures where appropriate and completes all position and unit safety related competencies and requirements on a timely basis.
Performs other duties as assigned.
Qualifications
Required
Associate’s degree in nursing
1 year of relevant experience preferably in Hospice
Michigan Registered Nurse (RN) license upon hire
CPR, (AHA) American Heart Association within 90 days of hire
Preferred
Bachelor’s degree
