Responsibilities

  • Administer prescribed medications and document administration per facility policy
  • Monitor residents’ health status, record vital signs, observe for changes, and report to the supervising RN
  • Provide basic wound care and dressing changes as ordered
  • Assist with activities of daily living, mobility, and transfers
  • Maintain accurate care documentation and participate in care planning with the RN and interdisciplinary team

Requirements

  • Active Licensed Practical Nurse license in New Hampshire
  • Graduation from an approved practical nursing program
  • CPR/BLS certification
  • Prior experience in long-term care or skilled nursing preferred
  • Ability to work evenings and thrive in a resident-centered, fast-paced LTC setting
  • Eligible to work in the United States
