Travel Licensed Practical Nurse Long Term Care Job

Responsibilities

• Administer prescribed medications and treatments per physician orders and facility policy
• Monitor residents’ conditions, document vitals, and report changes to the supervising RN
• Perform routine nursing assessments and wound care within scope of practice
• Document care and interventions accurately in the electronic health record; communicate with the care team
• Assist residents with activities of daily living while maintaining safety and comfort
• Collaborate with the interdisciplinary team to update care plans and respond to changing patient needs

Requirements

• Active Licensed Practical Nurse license in New Hampshire
• Long-term care or skilled nursing facility experience preferred
• CPR/BLS certification
• Ability to work 3x12-hour shifts and participate in a travel assignment
• Ability to pass background check and drug screen
• Graduation from an approved practical nursing program (LPN diploma/certificate)
