Responsibilities
- Administer prescribed medications and treatments per physician orders and facility policy
- Monitor residents’ conditions, document vitals, and report changes to the supervising RN
- Perform routine nursing assessments and wound care within scope of practice
- Document care and interventions accurately in the electronic health record; communicate with the care team
- Assist residents with activities of daily living while maintaining safety and comfort
- Collaborate with the interdisciplinary team to update care plans and respond to changing patient needs
Requirements
- Active Licensed Practical Nurse license in New Hampshire
- Long-term care or skilled nursing facility experience preferred
- CPR/BLS certification
- Ability to work 3x12-hour shifts and participate in a travel assignment
- Ability to pass background check and drug screen
- Graduation from an approved practical nursing program (LPN diploma/certificate)
