Registered Nurse (RN) - Home Care, flexible hours

The Registered Nurse (RN) Case Manager leads client assessments, care planning and caregiver oversight. This role involves collaborating closely with clients and their families to create personalized care plans and supervising the care provided by caregivers, to ensure that care provided meets state regulations, company policies, and client needs. Key responsibilities include conducting initial and follow-up in-home assessments, ensuring caregiver execution of care plans align with state regulations, serving as a liaison between families, caregivers, and office staff, and documenting care plans, assessments, and caregiver evaluations.
