RN Case Manager Job Description (Full-Time). Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. The RN role delivers skilled care in the home, advocates for patients and families, and helps raise the standard for home health. Responsibilities include patient assessments and skilled visit documentation including OASIS, delivering high-acuity home health care with a focus on wound care and infusion therapy, building and maintaining plans of care, serving as patient advocate, driving case management responsibilities, administering medications and treatments, providing supervision to LVNs and HHAs, and participating in case conferences.
