Our Visiting Registered Nurse (VNA) Certified division provides short- and long-term home care services for patients covered by Medicare, Medicaid, Private Insurance, and Private Pay. Services include Skilled Nursing, Physical Therapy, Occupational Therapy, and Home Health Aide support delivered in patients' homes. The RN serves as the primary clinical partner for patients and families, delivering individualized, one-on-one nursing care in the home that promotes recovery, safety, and independence. Conducts comprehensive in-home assessments, provides skilled nursing interventions including wound care, infusions, catheter care, medication management, post-operative care, and chronic disease management. Educates and empowers patients and caregivers, monitors patient progress, coordinates care across providers, and completes timely accurate documentation.
Requirements
Active RN License in the state of application
Current CPR certification (with hands-on component) or able to obtain one
Current TB test / TB skin test (current within last 12 months)
Driver's license, reliable transportation, and auto insurance
Must be able to read and write English
Foreign language proficiency (bilingual)
Preferred: Medicare Skilled Nursing experience
Preferred: Basic understanding of OASIS
