Senior Helpers of East Brooklyn is seeking a registered nurse for care plan assessments, reassessments, review, caregiver skills training, and agency compliance teamwork. Responsibilities include client care planning, reassessment every 6 months or less, compliance reporting, coordinating care with team, clients, families, and medical professionals, assessing client health status and reporting significant changes to healthcare providers, caregiver skills training at Center of Excellence, enforcing safety, infection control, and healthcare regulations, and maintaining accurate and timely documentation of care provided and deliverables.

Requirements

Valid New York State RN License
Valid license, registration, insurance for reliable personally owned vehicle
Experience in gerontological care in community, facility, or both settings
