About this role

Show full description
Guides patients through the healthcare system, establishing contact with patients, family members, and/or caregivers. Understands clinical pathways established by the clinical specialty and directs patients to health care services for timely diagnosis, treatment and survivorship. Identifies and addresses barriers to care. Assists patients and physicians in the transition from active treatment to continuing healthcare. Full-time, 40 hours per week, Monday-Friday with rotating call schedule at Atrium Health Wake Forest Baptist.

Requirements

  • Graduate from an accredited school of nursing, BSN required
  • Minimum two years' experience as a registered nurse in the specialty required
  • 3-5 years of nursing experience strongly preferred
  • Current licensure to practice as Registered Nurse in North Carolina
  • Certification as an RN preferred
  • BCLS required
  • Problem solving, decision making and critical thinking skills
  • Ability to work in a self-directed environment and with leading teams
